Levels of Care

Touchstone of Mayville is a community leader for assisted living, and known for it's exceptional care. We provide four levels of care for our assisted living home residents. Levels of Care and certain criteria affect placement and admission status. Each potential resident will be assessed and categorized by the following:

LEVEL 1 CARE:

Private room including housekeeping, laundry services (dry cleaning not included), three meals and daily snacks, scheduled activity program, monthly wellness check, 24 hour staff supervision; weekly stand by staff assistance with bathing, medication management, arrangement of local transportation, and cable television in every room.

LEVEL 2 CARE:

All services listed in level 1 care with the addition of any one of the following; staff assistance with transfers, with dressing, with grooming, with toileting, bladder incontinence assistanc

LEVEL 3 CARE:

All of the services listed in care levels 1 & 2 with the addition of any or all of the following services; diabetes management, therapeutic diets, individualized exercise regiment (PT support), mechanical lift transfer, two staff assist transfer, and Ostomy Care.

LEVEL 4 CARE:

All services listed in level 1, 2 & 3, with the addition of any or all of the following services; Complete assistance with ADLs (activities of daily living), hand feeding, catheter care, management of end of life services under the direction of hospice or home health agency.

Services that are not included in any of the levels listed above; personal telephone fees, personal toiletries, medications, DME (durable medical equipment), and incontinence supplies.

*Fees may be adjusted partially between levels as care levels demand.
